Your Passport Renewal Requirements

You answered “Yes” to Legally Changed Your Name and “Yes” to Over 18. Requirements are:

    1. Your expired/current US passport
    2. A copy of your name change petition issued by the local courthouse
    3. One standard 2×2 passport photo (Passport Photo Requirements)
    4. Typed US passport renewal application DS-11 (find the form here) DO NOT SIGN THIS UNTIL YOU GET TO THE PASSPORT ACCEPTANCE FACILITY
    5. Two signed authorization forms, put down “Walock Travel” as the courier company’s name
    6. Copy of your upcoming flight reservation (if you don’t have this available at this moment, please give us a call)

*Once you get to the passport acceptance facility, ask them for the HANDCARRY option by putting everything in a sealed envelope with the appropriate payment for the passport agency.
